Lorenzo Brino, who was a former child star, was reportedly killed when he lost control of his Toyota before it crashed into a pole, according to TMZ.
The incident occured on March 9 in San Bernadio County, but Brino's name was only confirmed today.
The young actor was best known for playing Sam Camden in the 90s drama 7th Heaven.
Lorenzo was on 7th Heaven from 1999 to 2007 and was first introduced to the show when he was just five-months-old.
The quadruplet along with his siblings, played twins Sam and David Camden, the family's youngest children.

According to reports, the actor was the only person present in the vehicle following the crash.
Lorenzo's friend Stephen Dulay called him a 'beautiful soul' in a tribute video to the former child actor.
In the video caption, he wrote: “From September 21st-March 9th 2020 we were blessed to have Lorenzo Brino’s beautiful soul in our lives.
“The amount of people he touched with his character and charm was unbelievable. He was a loyal friend and an amazing brother. He would always get my back no matter what. Rest In Peace Brother [prayer hands]."
Following the tragic news, his aunt Jane Brino, paid tribute to her “sweet nephew” in heartfelt message on social media, according to The Mirror.
She wrote: "To my dear sweet nephew, your loss left a hole in my heart. God needed another Angel and he took you.
"Please watch over your mom and dad. You sister Mimi, brothers, Antonio, Zachary and Nicholas ... You got a big job up there.”
Fans have since taken to social media to offer they condolences to the actor.
One person said: “RIP Lorenzo Brino......Never really know the Brino Quadruplets well but I mean, he was always a nice, genuine person when he has spoken to me, along with all his siblings. It’s tragic to go so soon, and unexpected. Never will be forgotten.”
Another Twitter user commented: “Just learned that one of the twins that played David and Sam on 7th Heaven passed away from a car accident so sad and only 21 rest in peace Lorenzo Brino.”
While a third person added: "This is just so sad."
Lorenzo appeared in a total of 138 episodes between the third and eleventh seasons of the show as Sam Camden.
